As a Revive VIP Maintain Member, you get a choice of a 90-minute Swedish Massage OR an exfoliating facial each month.

• Upgrade to a Hydrafacial for an additional $35 per treatment
• Upgrade to a DiamondGlow Facial for an additional $75 per treatment
• Enhance your upgrade by adding an Express Dermaplaning for an additional $50 in addition to the cost of the upgrade (either Hydrafacial or DiamondGlow).

Choose to pay monthly with an auto-draft or save and pay for the entire year.

Other terms:

• Revive VIP Maintain Membership treatments may only be redeemed by the member. If they are not redeemed by the member each month then spa treatments will be waived.
• $120.00 per month or $1200 per year (depending on the chosen option) will be automatically drafted each month from the member’s credit or debit card beginning on the date of enrollment and each subsequent month or year thereafter.
• Revive VIP Maintain Membership is automatically renewed annually at the end of the initial enrollment year and each year thereafter.
• Benefits can be canceled by the VIP Maintain Member after the initial year of membership is complete by giving written notice to Revive Medical Spa within 30 days prior to the annual renewal date.
• Revive Medical Spa, LLC Gift Cards may not be redeemed to pay any portion of the membership fee.
• The membership fee cannot be combined with any other promotion and/or discount or promotional gift card.
• Revive Medical Spa, LLC reserves the right to vary, add or eliminate any of the particular services and facilities provided from time to time.
• Revive Medical Spa, LLC reserves the right to close or modify facility hours with or without notice.
• Upon cancellation, all VIP member discounts and benefit will cease and not be applicable on any future purchases or appointments. Any money accrued will remain on account to be spent on products and services at full price.

Cancellation terms:

Please be advised that you have the right to cancel this Agreement:
a. If you are unable to utilize the membership due to (a) death (b) disability (c) illness (d) injury (e) a change in residence that is more than 30 miles from a participating Revive Medical Spa, LLC (f) your being called to active duty by the United States Military and deployed outside of the state of the primary Spa location or (g) if the membership and/or the primary Spa locations fails to open or permanently discontinues operations. In order to effectively exercise any right of cancellation, you must notify Revive Medical Spa, LLC of your intention to cancel in writing to Revive Medical Spa, LLC by email at info@revivemedicalspa.com or by mail to Revive Medical Spa, LLC, 1444 E. Stearns St., Fayetteville, AR 72703. b. Revive Medical Spa, LLC reserves the right at any time to cancel or suspend the membership of any member in the event monies due to Revive Medical Spa, LLC by the member remain unpaid for 30 days after its due date for payment.
