• Benefits can roll over month to month but expire after the initial enrollment term.

• Membership renews automatically at the end of the enrollment term, which is one year.

• Membership may be canceled after the initial term with 30-day written notice.

• Prices are subject to change.

Cancellation Policy

• Membership renews automatically at the end of the enrollment term, which is one year.

• Membership may be canceled after the initial term with 30-day written notice.

• Upon cancellation, member discounts and benefits end immediately. Any funds accrued in a savings-based membership remain on the account and may be used toward products and services at full price.

• Cancel in writing 30-Day Notice to Cancel.
• Fee charged unless proof is provided of move 50+ outside of the zip code where the office is located.

• Membership Fees are Non-Refundable.

• This membership fee recurs monthly unless cancelled prior to end of term.

Cancellation terms:
Please be advised that you have the right to cancel this Agreement:
a. If you are unable to utilize the membership due to (a) death (b) disability (c) illness (d) injury (e) a change in residence that is more than 30 miles from a participating Revive Medical Spa, LLC (f) your being called to active duty by the United States Military and deployed outside of the state of the primary Spa location or (g) if the membership and/or the primary Spa locations fails to open or permanently discontinues operations. In order to effectively exercise any right of cancellation, you must notify Revive Medical Spa, LLC of your intention to cancel in writing to Revive Medical Spa, LLC by email at info@revivemedicalspa.com or by mail to Revive Medical Spa, LLC, 1444 E. Stearns St., Fayetteville, AR 72703. b. Revive Medical Spa, LLC reserves the right at any time to cancel or suspend the membership of any member in the event monies due to Revive Medical Spa, LLC by the member remain unpaid for 30 days after its due date for payment.
